Demo reel shot breakdown

Not sure this is the right place to post this, but anyway, just a couple of quick questions really.

I've very recently completed my first demo reel (woohoo!) and am in the process of looking for potential employers. After reading countless threads on the subject of demo reels, I'm a bit worried about this 'shot breakdown sheet' thats required.

I'm sending my demo reel on vhs, so do I print it out on paper and just send it with my c.v?

I did ALL the work on my demo reel myself, so do I even need a shot breakdown sheet?

Would it be alright for me to just put something like 'All work done by Paul Gallagher' at the end of my reel?

I apologise for another demo reel thread, but I did a search and came back with nothing that really helped.

Shot breakdown is also a mean of showing HOW you did the shot. What are the steps, the layers, the render passes you used to acheive the final product? If it is animation, di you do a synoptic? An animatic? Did you plan your works. All this can be show in a shot break down.
